We provide a comprehensive range of services in the Home Health Care Service industry, tailored to meet the unique needs of our clients.
Adult Foster Care Program (AFC)—is a program that allows persons above the age of 16 who need assistance with personal care tasks to remain in their home and community with a qualified and approved caregiver.

Group Adult Foster Care
Provides help with daily care such as bathing, dressing, and moving safely. Services include medication reminders, meal preparation, light housekeeping, and regular nurse check-ins. MassHealth covers the cost for eligible members.
Help with activities of daily living such as bathing, dressing, grooming, etc.
Personal Care
Member Eligibility
Must be 16 years or older
Require assistance with at least 1 ADL (Activity of Daily Living)
Must have MassHealth Standard
Must have a family member (can’t be spouse or parent, if the member is a minor or guardian) who lives in the same home.
Caregiver Eligibility
Caregiver must be 18 years or older
Can be a family member or friend (excluding spouses)
Complete required training and orientation.
Must pass CORI, SORI, OIG check
Must have TB Screening within the past 12 months
