How we review your application
No black box. Here is exactly what our intake team does with the details you send.

We verify MassHealth coverage
We check which plan the member is on and whether it supports AFC or GAFC. Qualifying GAFC coverage may include MassHealth Standard or CommonHealth.

We check the living arrangement
AFC requires the member and the paid caregiver to live together in a private home. A spouse or legal guardian cannot be the paid AFC caregiver. If co-residence isn't possible, we look at GAFC instead.

We confirm the service area
We check that our nurses and care managers can reach the home for regular visits, and tell you honestly if we can't.

We arrange the nurse assessment
If it looks like a fit, a registered nurse visits to document the level of daily care needed, and we pursue any required authorisation.
About eligibility and timing. Many families may qualify. Eligibility depends on MassHealth coverage, documented care needs, living arrangements, program rules, and any required authorization. Coverage and clinical eligibility are subject to MassHealth and health-plan verification.
