If you're the one showing up every day for a parent, adult child, sibling, other relative, or friend, you don't have to do it unsupported and unpaid. Through Adult Foster Care, eligible caregivers get certified, backed by a nurse, and compensated for the care they already give. A spouse or legal guardian cannot serve as the paid AFC caregiver.

You’re trained for it, a registered nurse guides you, and you receive a stipend for your time that is generally excluded from federal taxable income when applicable requirements are met.

What it takes to qualify
The bar is about dependability and care, not a résumé — though MassHealth does set firm rules on co-residence and who may be paid. If this sounds like you, it's worth a conversation.
- Be at least 18 years old and legally able to work in the U.S.
- Live together with the member in the member's or caregiver's private home
- Not be the member's spouse or legal guardian
- Pass a background check (CORI) and reference review
- Complete caregiver orientation and hands-on training
- Be reliable, patient, and committed to the member's wellbeing
