Looking to be paid for caring for your own family member?

That’s a different path, and it isn’t agency employment. Through MassHealth Adult Foster Care, an eligible relative or trusted friend who lives with the member can be trained, supervised, and paid a stipend to provide their daily care.

These are the positions we recruit for most often. Openings vary by season and caseload — if your background fits one of these, send your résumé even if nothing is posted.

Full-time & part-time

GAFC Direct Care Worker / Home Health Aide

Provide scheduled personal care to members in approved senior and supportive housing communities. Bathing, dressing, mobility, medication reminders, and companionship, all within a nurse-approved plan of care.

What we look for

  • HHA or CNA certification preferred; we will train the right candidate
  • Reliable transportation within our service area
  • Ability to pass a CORI background check
Full-time & per diem

Registered Nurse (RN)

Complete in-home assessments, build and review plans of care, oversee caregivers and aides, and coordinate with members' physicians across AFC and GAFC caseloads.

What we look for

  • Current, unrestricted Massachusetts RN license
  • Home care, geriatric, or community health experience valued
  • Comfort working independently in members' homes
Full-time

Care Manager

Be the steady point of contact for families. Coordinate enrollment, monitor how care is going, respond when circumstances change, and keep documentation accurate and current.

What we look for

  • Social work, case management, or human services background
  • Strong documentation habits and follow-through
  • Bilingual English/Spanish strongly preferred
Full-time

Intake & Referral Coordinator

Answer inquiries from families and referral partners, verify MassHealth coverage, gather documentation, and move referrals through enrollment without anyone falling through the cracks.

What we look for

  • Healthcare administration or medical office experience
  • Familiarity with MassHealth or Medicaid processes valued
  • Bilingual English/Spanish strongly preferred
