For members age 16 or older who live in a private home with the caregiver who will be paid to provide their daily care.

Possibly. The towns listed here are where we most often serve families, but coverage changes as our care team grows. Call us with your town and we'll tell you honestly whether we can staff it or refer you to a provider who can.
Do you serve all of Massachusetts?
No. We concentrate on western and central Massachusetts, where our nurses and care managers can reach members reliably. If you're outside that area, we're glad to point you toward MassHealth's provider directory.
Does the service area differ for AFC and GAFC?
It can. Adult Foster Care is delivered in a private home, so coverage depends on whether our nurses can reach that home for regular visits. Group Adult Foster Care also depends on whether the specific housing community is approved to offer GAFC. Tell us where your loved one lives and we'll check both.
